A diverse non-profit, community-based organization is seeking to fill Missouri Bootheel Healthy Start grant funded positions.  These positions will work out of Sikeston, MO office.  Services are provided to clients and volunteers in Dunklin, Pemiscot, New Madrid, Scott and Mississippi counties.  Excellent benefit packages available for Full-time and Part-time employees that include: major medical, dental, vision, short term disability, life insurance, paid time off and ten paid holidays. Clinical Case Manager Supervisor

Provides direct case management services for high risk pregnant and interconception women, infants and families with a minimal case load and supervise case management staff. Function as a member of the Case Management Team and oversee services and monitor program quality.  LPN/ RN, BSN with OB and mental health, experiences preferred. Experience in case management visitation, public health education, prenatal, infant health and Medicaid preferred A minimal of two-three years supervisory experience preferred. Community outreach/group facilitation, volunteer experiences and knowledge of the Bootheel a plus. Demonstrates Microsoft office computer proficiency, technical writing skills and have knowledge of HIPAA compliance.   No on call night and weekend work. Full time position with salary range low to mid 40’s.

 

 
 

Case Manager

 

Provides case management and service coordination for pregnant and interconception women, infants and families, who reside in Scott, Mississippi, Pemiscot, Dunklin and New Madrid counties. Desire to work with people of diverse background and life experiences and function as a member of the Case Management Team.  Demonstrates community outreach experience and have knowledge of HIPAA compliance.  Good oral and written communication skills, Microsoft office computer proficiency, data entry, counseling techniques and problem-solving skills desired.  Bachelor degree required in areas of medical, social work or mental health.  Additional education or knowledge in criminal justice and health care a plus. Two- three year experience in case management-in home visitation, prenatal, infant health, Medicaid and knowledge of the Bootheel preferred. No on call night and weekend work. Full time position with salary range low 30’s.

Perinatal Health Educator

Provides prenatal and health education and trainings to high risk pregnant and interconception women and families in the Healthy Start program. Provides trainings and education to program participants, community groups and providers. Experience working in women’s health with diverse backgrounds and high risk families a plus. Serves as primary facilitator in perinatal education activities. Manages coordination and development of education resources for program and ability to be an enthusiastic team player.  LPN or Bachelor’s degree with major emphasis on Community Health Education or Public Health desired. Two or more years’ experience working in Public Health or with Maternal, Child or Family Health, Public Relations, Parenting Education, Mental Health, Community Outreach, and Marketing preferred. Ability to assess and evaluate educational needs of program and knowledge of the Bootheel a plus. Full time position with salary range low to mid 30’s.

 

Vehicle Driver

On call Driver will be responsible for transporting healthy start case management clients and children to and from prenatal provider visits and program participants to meetings and events. All routes will begin in Sikeston and will service Scott, Pemiscot, Mississippi, Dunklin and New Madrid counties. Functions as an appropriate role model and requires friendly and good customer service skills. The driver may participate in distributing flyers for program activities. Applicants MUST pass pre-employment drug and alcohol screening and background checks including: State of Missouri Criminal, elderly abuse, and child abuse check. A class E chauffeur’s driver’s license is required. This position is not eligible for benefits. Part-time position with salary range of $10.00 per hour.

Community Outreach Worker
The position will be responsible for recruiting a specific number of women in their first trimester into the Healthy Start program. Having knowledge of Healthy Start programs and volunteering a plus. Current and previous Healthy Start Case Management clients and volunteers are encouraged to apply.  Position requires flexible hours and reliable transportation with valid driver's license. A friendly and outgoing personality is a must.   High school Diploma or GED required. Position is part time, salary $12.00 a hour with benefits.
